This package should be used when a great-grandparent, grandparent, aunt, uncle, niece, nephew, or sibling is adopting a minor relative. The petitioner must obtain the written consent of the child(ren) to be adopted, if the child(ren) is/are twelve years of age or older, as well as the written consent of the child(ren)’s parents or legal guardian.
